Student Activity Request Forms
Student Leadership & Activities forms are to be completed electronically and returned to the Office of Student Leadership and Activities on your campus. Please consult their webpage for information and instructions on how and where to submit your forms.
- New Club/Organization Charter Form - Apply to start a new student club or organization.
- Event/Facility Request Form - Request approval for club meetings, fundraisers, volunteer activities, or other events.
- Food Order Form - Request food for your club meeting or event (costs may be covered by your SLA funds).
- Fundraising Proposal Request - Get approval to host a fundraiser.
- Deposit Form - Use this form to deposit money raised from your fundraiser into your club account.
- Activity Reporting Form - Report completed club meetings, fundraisers, or volunteer activities.
- Release of Liability Form - Required for any club travel.
- Combined Travel Waiver and Transportation Election Form - Submit before traveling with your organization.
- Check Request Form - Request a check or bulk purchase using club funds.

ARTICLE I: NAME OF CLUB

ARTICLE III: MEMBERSHIP

ARTICLE IV: QUALIFICATION AND ELECTION OF OFFICERS
Section 1. To qualify to be an officer the student must currently be enrolled, have a minimum of a ________ G.P.A, and must not be on academic or social probation.
Section 2. Officers will be a President, Vice-President, Secretary, and Treasurer.
Section 3. The election will be done by secret ballot by club members.
ARTICLE V: LIST OF OFFICERS AND DUTIES
Section 1. President
A. Preside over all meetings.
B. Call special meetings.
C. Carry out the provisions of the constitution.
D. Appoint committees and chairpersons.
E. Oversee all committee activities.
F. Attend or appoint an Inter Club Council Representative.

Section 2. Vice President
A. Assume the duties of the president in his/her absence.
B. Perform any duties delegated by the president.
C. Report the results of ICC meetings at club meetings.

A. Record and keep accurate minutes of all meetings.
B. Act as correspondence clerk.
C. Print and distribute agenda for all meetings.

A. Handle funds and finances for club.
B. Keep financial records and collect dues.
C. Pay bills and release funds as voted by the general membership.
D. Make financial reports at least once a month at the meeting.

ARTICLE VI: IMPEACHMENT/REMOVAL AND REPLACEMENT OF OFFICER AND MEMBERS
Section 1. All elected officers and club members may be subjected to impeachment and removal by a two-thirds majority vote of the total membership.
Section 2. Grounds for impeachment are negligence and any form of misconduct which is damaging to the club. An officer/member may be impeached/removed only under the following conditions: there must be a quorum present during impeachment/removal; two-thirds of the membership must vote; prior to impeachment/removal there must be one week's notice of intent publicized.
Section 3. Any officer vacancies shall be filled by an election held within two weeks.
ARTICLE VII: MEETINGS
Section 1. There must be a meeting at least once a month held at Blinn College.
Section 2. The regular meetings for this organization shall be held (check one):

Section 3. There must be a quorum (a quorum is 50 percent plus 1 of the active membership) present in order to have an official meeting.
ARTICLE VIII: AMENDMENTS TO CONSTITUTION
Section 1. Any amendment change requires a two-thirds vote at a general club meeting and must then be approved at the Inter Club Council Meeting.
Section 2. A copy of this constitution/by-laws and/or amendments to this constitution must be submitted to the Office of Student Leadership and Activities.
