Disability Services Forms
Appointments are available Monday through Friday during normal business hours. The office will respond to your request at our earliest convenience.
Disability Services intake and letter requests are only monitored during regular office hours (Monday through Friday, 8:00 a.m. to 5:00 p.m.). Requests submitted after 5:00 p.m., on weekends, or during Blinn holidays will be responded on the next Blinn normal operation business day.
