Brenham Campus SLA Forms

We invite you to become familiar with the resources, guidelines, and procedures outlined in the Club Organizational Manual. Whether you are starting a new organization, planning an event, or managing an existing club, the manual provides helpful information to guide you along the way.

Please complete both forms below. Both forms must be printed and completed to apply for a new student club or organization.

After completing both forms above, upload them using the New Club Request Form linked below.

• Event/Facility Request Form
  Request approval for club meetings, fundraisers, volunteer activities, or other events.
• Indoor Tabling Reservation Request-Brenham
  Indoor tabling is reserved for registered student organizations, registered employee organizations, Blinn College Departments, and government entities. Any outside non-profit organization that wants to use an indoor table must be sponsored by a student or employee organization, or a College Department. Please review the Indoor Tabling Guidelines Administrative Regulation for more information.
• Food Order Form
  Request food for your club meeting or event (costs may be covered by your SLA funds).

Note: If your event is a fundraiser, you must also complete the  Fundraising Proposal Request (see below).

• Fundraising Proposal Request - Get approval to host a fundraiser.
• Deposit Form - Use this form to deposit money raised from your fundraiser into your club account.
• Event/Facility Request Form - Required for the day of the fundraiser to confirm your location and setup.

• Release of Liability Form - Required for any club travel.
• Combined Travel Waiver and Transportation Election Form - Submit before traveling with your organization.
