Application for LicensureThe Texas Board of Nursing approves students to take the licensure exam. Certain criminal convictions prohibit applicants from applying to take the exam. Acceptance to the program does not guarantee graduation or approval to take licensing exam. The Texas Board of Nursing determines eligibility requirements for applicants for the initial licensure by examination. If eligibility issues arise, the eligibility process takes a minimum of 3-6 months. All candidates for licensure will be required to answer the following questions: 1. Have you been practicing as a licensed vocational nurse within the past four (4) years? 2. Have you ever held a Texas Licensed Vocational Nurse License? 3. For any criminal offense, including those pending appeal, have you:
4. Are you currently the target or subject of a grand jury or governmental agency investigation? 5. Has any licensing authority refused to issue you a license or ever revoked, annulled, cancelled, accepted surrender of, suspended, placed on probation, refused to renew a license, certificate or multi-state privilege held by you now or previously, or ever fined, censured, reprimanded or otherwise disciplined you? 6. Within the past five (5) years have you been addicted to and/or treated for the use of alcohol or any other drug? 7. Within the past five (5) years have you been diagnosed with, treated, or hospitalized for schizophrenia and/or psychotic disorder, bipolar disorder, paranoid personality disorder, antisocial personality disorder, or borderline personality disorder? If “YES” indicate the condition: [ ] schizophrenia
and/or psychotic disorders, [ ] bipolar disorder, NOTE: If you answered “YES” to questions 1 - 7 listed above, attach a letter of explanation that is dated and signed indicating the circumstance. Attestation/Consent to Release & Use of Confidential
Records If your response is “yes”
to any of these questions, you MUST submit a petition for “Declaratory
Order” to the Texas Board of Nursing prior to enrollment or within
the first semester of the program. For information or guidance in this
process to determine eligibility for licensure by examination, contact
the Texas Board of Nursing at 512-305-7400 or go to the web site: www.bne.state.tx.us.
Applicants with any of the above situations are advised that admission to and graduation from this program does not automatically guarantee that the Texas Board of Nursing will allow licensure. The Board will not approve an applicant to take the NCLEX-PN or issue a permit to practice as a Graduate Vocational Nurse until an eligibility decision has been rendered. If eligibility issues arise, the eligibility
process takes a minimum of 3 - 6 months. The STUDENT is responsible for
