Required Immunizations

Varicella (chicken pox) - Must have proof of either:

a physician-documented history of the disease
documentation of two varicella immunizations
a serum titer confirming immunity
parent or guardian certification statement that you had chicken pox as a child
Note: The varicella injection series is a four-week process

Hepatitis B - Proof of either: (MUST BE COMPLETED PRIOR TO ADMISSION)

a complete three-injection series of hepatitis B vaccinations
a serum titer confirming immunity
Note: The hepatitis B injection series is a 6 month process

Measles, mumps, and rubella (MMR) - Proof of either:

an MMR vaccination
a serum titer confirming immunity to each disease
Note: Students born before January 1, 1957 are exempt from the MMR requirement

Measles - Proof of either:

a second measles vaccination (may be part of a second MMR)
a serum titer confirming immunity
Note: Students born before January 1, 1957 are exempt from the measles requirement
Note: There must be at least one month between the first and second MMR vaccination

Tetanus - Proof of tetanus vaccination within the last 10 years.

Tuberculosis Screening Test - A PPD skin test to screen for TB is required before entry into program (or a chest x-ray if student is unable to have a PPD)

Note: All immunizations are at the student's expense.
