COURSE DESCRIPTION: Introduction
to the systems and processes for collecting, maintaining, and disseminating
health related information. Instruction in the delivery and organizational structure
including content of health records, documentation requirements, registries,
indices, licensing, and regulatory agencies. Three class hours per week. Credit:
Three hours.

CRITERIA FOR GRADING
ALL ASSIGNMENTS AND DETERMINING FINAL GRADE IN THE COURSE:
A. Major Exams: Score determined by points earned on objective and written answers.
B. Assignments: Score determined by points earned for completion of assignments.
C. Individual project/presentation: Score determined by points earned.
D. Daily grade: Points determined by various activities, ex. Participation in
activities and classroom discussion, quizzes.
E. Final Exam: Points determined by accuracy. Points also given for correct
format, and following all instructions.

INSTRUCTORS POLICIES:
A. Attendance: Refer 2007 Blinn Attendance Policy
Students are expected to attend all classes. Internet students are expected to make and maintain regular contact with the instructor during the course. An accurate record of each student’s attendance is kept by each instructor. A student who is subject to the THEA rules and who acquires the equivalent of two weeks of absences in the required developmental course will be withdrawn from the college. A student on scholastic or disciplinary probation should not have any unexcused absences. Students are not assessed absences when representing Blinn College. Class days missed due to inclement weather will be made up as appropriate. It is the student's responsibility to officially drop a class he or she is no longer attending. To officially drop a class the student must contact an admissions office to obtain the class withdrawal from an admissions office, complete the class withdrawal form, secure the required signatures and return the completed form to an admissions office. Drop date deadlines are posted in the Blinn College Calendar. Failure to properly withdraw will result in a grade of F being issued.
B. Make-up work: Students are expected to be in class each day, but especially on major exam days. Students not in attendance and taking the exam on the scheduled day will be allowed a make-up exam with a 10% penalty for the first exam missed, a 20% deduction for the second time a major exam is missed, and a 30% deduction for the third time a major exam is missed. The make-up exam may or may not be the same as the regularly-scheduled exam, and could contain more problems. The make-up exam must be scheduled with the instructor and must be made up before the next exam.
C. Dishonesty of any kind will not be tolerated. This includes copying work from another student or student computer diskette and turning it in as your own. Depending on the severity of the dishonesty, one may receive either a zero for the assignment or fail the course at the discretion of the instructor. All persons involved in the copying or granting permission for copying will be punished.
D. Housekeeping. Please keep work area neat at all times. At the end of the period, please pick up and throw away any waste paper, and put chairs back in proper place. No food or drinks in classroom.
E. Final Exams are given at the time stated; late-comers must complete their exam by the time of the stated exam ending; students absent the day of the final exam will lose 200 points allotted for the exam.F. ADA Statement: Refer Blinn ADA Statement
