Request Counseling
The first step in arranging for a consultation is to complete an intake questionnaire, either online or by printing a pdf file (152 KB ~ opens in new window) and mailing it to us. If you prefer, you may call us at (979) 830-4137 and we will send the questionnaire to you.
The information you provide will help us assess your individual needs. It will be held in strict confidence as will all proprietary information discussed during your counseling sessions. After completing the questionnaire form and submitting it, you will be called and a recommendation will be made for training and/or counseling.
If you anticipate a need for financing and do not have recent credit reports, we suggest that you obtain reports from the 3 major credit bureaus. We have compiled instructions for obtaining credit reports. Please bring your credit reports along to your first appointment.
