Oral Interpretation Documentation Forms

We host four district and two regional meets
each spring; each meet has upwards of forty-eight oral interpretation contestants,
each with two sets of documentation. Your completing these forms correctly
and your providing the correct documentation--and nothing more!--faciliate
the smooth running of these contests. Your careful attention is indeed appreciated.
Please link to the appropriate Oral Interpretation Documentation Form according to its intended use at either district or regional meet.
Please note that each contestant MUST be represented by TWO completed forms (each accompanied by appropriate documentation)--one set of form-plus-photocopied documentation for each of the categories in which he or she might compete.
Completed forms and required photocopied documentation are due at the contest site no later than four days before the date of the meet.
Contestants advancing from District 27-A, District 28-A, District 18-AAA, and District 23-AAA competition AND who are not changing their selections do not have to resubmit forms and documentation. A note from the contestant's coach indicating that the same material will be performed at the regional contest, faxed to the same number, will suffice.
