Faculty Self-Assessment

The purpose of the faculty evaluation process is to improve instruction. The student evaluations, supervisor's evaluation, and self-assessment instrument provide for a wide range of input and feedback to be used in clarifying and improving teaching goals.

Use information from the student evaluations, supervisor's evaluation, and your own observations and experiences in completing this form. Retain one copy for your files, and provide your immediate supervisor with a copy. The supervisor may wish to discuss this document with you. The purpose of the discussion will be to commend strengths, identify areas for growth, and focus on planning for improvement.

A. Professional Growth

1. List and describe your participation in student, division, and/or institutional activities during the current academic year. For example: Do you sponsor a student organization? What committees have you served on within your division? What projects are you working on within the division? What standing committees have you served on at the college level? Are you currently a member and/or an officer in a college organization?
 
 
 
 
2. Summarize activities related to professional growth. For example: Are you currently a member and/or an officer in a state, regional, or national professional association related to your teaching discipline? Which professional conferences or workshops have you attended during the past twelve months? Have you completed graduate courses related to your teaching discipline during the past twelve months?
 
 
 
 
B. Discuss the goals you developed for this academic year and how successful you were in accomplishing these goals.
 
 
 
 

C. Evaluation Response

1. Respond to the student evaluations of your classes by identifying areas of strength and areas that require improvement.
 
 
 
 

2. Respond to the supervisor's evaluation.
 
 
 
 

D. List and discuss the goals you have developed for the next academic year that will enhance your personal knowledge of course content, introduce new teaching methods, etc., or in some way improve your classroom instruction.
