- Completed application and $40.00 application
fee sent to Misti Corn, TEACH Certification Officer
- Three letters of reference sent to Theresa
Wahrmund, TEACH Director
- Official scores at the required minimum
levels from ACT, SAT, THEA/TASP, or Accuplacer
- Official transcripts (**mailed
to Misti Corn, TEACH Certification Officer) from EACH
college/university attended. (To insure that transcript information
is available, you may send unofficial transcripts until official
are received). **Please note that transcripts sent
electronically cannot be accessed by our office due to privacy
issues**
