Blinn Athletes and Prospective Athletes
This page is dedicated to helping athletes make sure that they have all of the paperwork that they need in order to participate at Blinn College. All athletes must have these forms read and signed before they are allowed to participate in any type of athletic activity. If you have any questions about these forms please call Kris Kapchinski at 979-830-4175. These forms can be hand delivered to the athletic training office in room 101 of the PE Building on the Brenham campus. These forms may also be faxed in to 979-830-4032.

Medical history form
Athletes must also provide a copy of any health and dental insurance that they have. Non-scholarship athletes MAY NOT participate unless they can provide proof of health insurance. If you are a non-scholarship athlete and do not have insurance you may contact any health insurance broker to pay for medical coverage. Health insurance companies include but are not limited to Blue Cross Blue Shield and United Healthcare.
